In caring for their patients and supporting SDMs to make health care decisions at a very difficult time, Brenda, Lisa, Katie and Lynn bring a wealth of experience in supporting and talking with patients and families about making health care decisions at a very emotional time. Their unique view at the beside is exactly what we need to take this exploration of ACP further.

When Lynn Weimer, an ACP ambassador and retired ICU nurse, suggests that “you need to talk about more than just surgeries, medications and treatments” she challenges all of us to think about what is most important to us. What are those priorities for LIVING that can influence health care decisions? What happens when you can’t decide for yourself? OR what happens when you haven’t told your SDM what you value most?   

Using a Q&A format, Katie, Brenda, Lisa and Lynn will share what they have seen in practice, their own observations about choosing/being someone’s SDM, as well as what to consider in preparing the SDM to make decisions on your behalf.
