Advance Care Planning has received unprecedented attention in the media as a result of COVID-19 and has highlighted the importance of planning ahead, sharing your wishes for health care in the event you are too ill or injured to speak for yourself.   It’s never too late–until it is–and yet we tend to procrastinate about having these conversations, often until it is too late.  Join us as we take another step forward in exploring these conversations, remembering that these talks with our Substitute Decision Maker (SDM) are about love and about how we care for each other. You don’t have to make your list of preferred treatments, instead we explore what is important to you so that your SDM(s) has the information needed to act on your behalf.

Our own team of ACP Ambassadors bring a wealth of personal and professional experience to this topic and we hope that our discussion will explore many of the questions you have about ACP conversations.  

  • What do I need to share with my SDM in order to prepare them?
  • What is most important to me, what do I value that could guide someone else to make decisions on my behalf?
  • How do I start these conversations?
